Employee Status Change

There are times when an employee will experience a change in status, which may result in a change of coverage or the need to add/delete dependent(s).  Contract changes can only occur during certain events, such as:

  • Marriage
  • Birth
  • Adoption
  • Divorce
  • Death
  • Loss of Coverage

 

With any of the above events, the employee has a thirty (30) day eligibility period to make changes to their current health care plans. The employee must provide Human Resources with all the required completed paperwork within this eligibility period. If the employee misses the eligibility period deadline, they must wait until annual open enrollment to make the health care plan changes.

Marriage

An employee has a thirty (30) day eligibility period to make changes to their current health care plans. The employee must provide Human Resources with completed paperwork and the required documentation within the eligibility period to start the review process. If the employee has missed the eligibility period deadline, they must wait until annual open enrollment.

Recommended paperwork to be completed:

 

Required documentation:

  • A copy of a certified marriage license is required to process the health care changes

 

Effective date of change:

  • Health care benefits are effective the date of marriage, please allow 7 to 10 business days for processing
  • Tax changes go into effect the pay period following Human Resources receipt of the necessary tax forms

 

Health care cost:

  • As an employee, there may be an additional cost for health care to add a new spouse or dependent(s) onto any of your health care benefits.  Please call Human Resources, Benefits Division at 469-5650 for more information

Birth

An employee has a thirty (30) day eligibility period to make changes to their current health care plans. The employee must provide Human Resources with completed paperwork and the required documentation within the eligibility period to start the review process. If the employee has missed the eligibility period deadline, they must wait until annual open enrollment.

Recommended paperwork to be completed:

 

Required documentation:

  • A copy of verification of birth is required to process the health care changes. Please DO NOT wait for the certified birth certificate or social security number. You have only 30 days to complete the paperwork and the birth certificate may take longer than 30 days.  Social Security number is required once available

 

Effective date of change:

  • Health care benefits are effective the date of birth, please allow 7 to 10 business days for processing
  • Tax changes go into effect the pay period following Human Resources receipt of the necessary tax forms

 

Health care cost:

  • As an employee, there may be an additional cost for health care to add a new child onto any of your health care benefits. Please call Human Resources, Benefits Division at 469-5650 for more information.

Adoption

An employee has a thirty (30) day eligibility period to make changes to their current health care plans. The employee must provide Human Resources with completed paperwork and the required documentation within the eligibility period to start the review process. If the employee has missed the eligibility period deadline, they must wait until annual open enrollment.

Required documentation:

  • A copy of the official adoption papers is required to process the health care changes
  • Social Security number is required once available

 

Effective date of change:

  • Health care benefits are effective the date of the final adoption takes place, please allow 7 to 10 business days for processing.  Tax changes go into effect the pay period following Human Resources receipt of the necessary tax forms

 

Health care cost:

  • As an employee, there may be an additional cost for health care to add a new dependent(s) onto any of your health care benefits. Please call Human Resources, Benefits Division at 469-5650 for more information

Divorce

An employee has a thirty (30) day eligibility period to make changes to their current health care plans. The employee must provide Human Resources with completed paperwork and the required documentation within the eligibility period to start the review process. Failure to notify Human Resources within 30 days from the date of divorce, the employee may be responsible to pay loss premiums.

Recommended paperwork to be completed:

 

Required documentation:

  • A copy of the Judgment of Divorce is required to process the health care changes

 

Effective date of change:

  • Health care benefits are canceled as of the last day of the month in which the divorce is finalized, please allow 7 to 10 business days for processing.  Tax changes go into effect the pay period following Human Resources receipt of the necessary tax forms

Loss of Coverage

An employee has a thirty (30) day eligibility period to make changes to their current health care plans. The employee must provide Human Resources with completed paperwork and the required documentation within the eligibility period to start the review process. If the employee has missed the eligibility period deadline, they must wait until annual open enrollment.

Recommended paperwork to be completed:

 

Required documentation:

  • A letter from the employer or insurance carrier is required to support the loss of coverage status before health care changes can occur

 

Effective date of change:

  • Health care benefits are effective the date the coverage was lost, please allow 7 to 10 business days for processing

 

Health care cost:

  • As an employee, there may be an additional cost for health care to add a spouse or dependent(s) onto any of your health care benefits. Please call Human Resources, Benefits Division at 469-5650 for more information
